Steps to Document Injuries

  1. Record Details of Affected Personnel Capture specific information about individuals involved in the incident.

  2. Describe the Nature and Severity of the Incident Provide a comprehensive overview of the incident's characteristics and its impact.

  3. Notify Authorities and Document Response Actions Ensure relevant authorities are informed and document any response measures taken.

Step By Step Guide

1. Click "Site Management" on the Plexa Dashboard

2. Click "Safety Management" in the Site Menu

3. Click "Injuries "

4. Click "Create Injury"

5. Enter ALL Injury Details in Section 01 and Select the "Injured Personnel"

6. Click "Next"

7. Select the relevant body part that has been injured eg right thumb.

8. Select "Reasons" that contributed to the Injury from the list

9. Tag additional Personnel Involved or Witness to the Injury

10. Select the Personnel from the Checkbox

11. Click "Save Personnel"

12. Tag additional SDS, Equipment and SWMS that were involved or applicable

13. Click "Next"

14. Use the "+ Drag & Drop or Browse" option to add attachments

15. Click "Next"

16. Enter details in Section 4 of the Injury Report

17. Click "Finish"
